I understand that martial arts and fitness activities involve risk of injury and agree to participate at my own risk. I confirm that I am in good health. I release Rising Star Martial Arts Academy, its staff, and affiliates from any claims or damages related to my participation or use of the facilities. I agree to follow all rules and I understand that failure to do so may result in removal. I authorize Rising Star to use my photo or voice for promotional purposes without compensation.
